Don't have PDF reader to use to print?
There are different ways to send order form to SAMPUBCO.
Print out order form. Do any one of these steps:
Option A: Get your smartphone (Android, IPad, etc) out, take a photo of filled order. Send by email attachment, (see the paper clip
to dsam52 at sampubco.comOption B: Use scanner, save as PDF or jpg, send order form by email attachment to dsam52 at sampubco.com
Option C: Save the order form (in .txt) in your computer, go to notepad, wordpad or whatever that will open as text format.
Fill in and save. Send by email attachment to dsam52 at sampubco.com
RTF (rich text format) forms can not be accepted due to different codes used in different programs.

If you are going to add images to your Tree in FamilySearch, Ancestry or MyHeritage, check both images and PDF if desired.
Currently Findmypast is not accepting PDFs for their Family Trees.

Paypal handle all credit transactions very safe and secured. SAMPUBCO does not see any credit card numbers at all.

$6.00 per record, PayPal or Credit /Debit Card / eCheck (if card or eCheck, please request invoice)
You are not required to have PayPal account to order anything.
You can use credit/debit card or eCheck, please request invoice
$8.00 per will record, check (via slow mail)
Paper Copy:
Paper Copy: Not available. Scan and/or PDF only.
Paper copy is not available for the probate files due to unknown number of pages/images in each file.
It is not practical.
